If you are using your TakeCare® Debit Card...
1. Simply present your Take Care® Debit Card at the time of payment to your provider.
2. Next, obtain an itemized receipt from your provider (Receipt must include the provider’s name, date of service, patient’s name, services provided, and the amount of the charge, or the Explanation of Benefits from your insurance carrier. For prescription drugs, include the prescription drug tax receipt which includes the date of purchase, the name of the patient, the drug name, and the name and address of the pharmacy). For over-the-counter drugs include the itemized cash register receipt.
3. If your charge matches your employer’s health plans copay you should not need to take further action. If your charge does not match your employers health plan copay you will need to attach your receipts to the Take Care® "Receipt Transmittal Form" and mail or fax to EBS/Atlanta.

If you are filing a manual claim...
1. Pay for the services provided at the time of your visit.
2. Next, if you are covered under an HMO or POS plan, obtain an itemized bill from your provider at the time of your visit. If you are covered under an indemnity or any other health plan where the expense is applied to a deductible, submit your expense to the insurance carrier first, then, send a copy of your Explanation of Benefits (EOB) form to EBS/Atlanta. For prescription drugs, include the prescription drug tax receipt (which includes the drug name). For over-the-counter drugs include the itemized cash register receipt.
3. Then, attach your receipts to the “Claim Form” for submitting manual claims. Mail or fax your claim to EBS/Atlanta.
