The Take Care® Debit Card from EBS/Atlanta makes your Health Care Flexible Spending Account and Dependent Care Spending Account easier to use. The Take Care® Debit Card is a stored value VISA® card that draws on your FSA election amount. Each time you incur a qualified expense at a qualified provider that accepts VISA®, simply present your Take Care® debit card for payment. It’s easy to use!
The Take Care® Debit Card is designed to work at Health Care Providers, Pharmacies, Chiropractors, Dental and Vision offices. The card will not work at locations other than medical providers, such as a gas station or restaurant. You should make sure that you only use the card for eligible expenses covered under your Flexible Benefits Plan.
Here’s how it works:
Once you use your card, remember to hold onto your receipts. The IRS requires that only those expenses that are covered through your employers health plan, such as physician copay’s, pharmacy copay’s and dental and vision copay’s, will not require the participant to submit receipts for documentation, as long as they “match” the copay designed by your employers health plan. Keep in mind, that if you are at the pharmacy paying for multiple prescriptions, or using a mail order pharmacy benefit program for multiple prescriptions at one time, you will need to submit all your prescription drug tax receipts regarding these transactions to EBS/Atlanta. Purchasing a single prescription that matches your employer’s pharmacy copayment’s, will not require you to submit a receipt to EBS/Atlanta. If you are using your Take Care® Debit Card at a dependent care provider, remember once your transaction has been approved, and your expense has been paid with your Take Care® Debit Card, you will still need to submit receipts for these expenses (as required by the IRS). EBS/Atlanta will notify you in writing when a receipt is required. If you prefer to send a receipt that does not match your copay in advance, you may do so by submitting them with the “Take Care® receipt submittal form”, which may be obtained on our website at www.ebsatlanta.com or by calling 1-800-647-3709.
Should the Take Care® debit card be used for any unauthorized use of ineligible expenses as according to the IRS rules and regulations, you will be required to “pay back” your account for such expenses. If you receive notification from EBS/Atlanta requesting documentation, remember to submit your receipts as soon as possible. As according to IRS regulations, we are required to obtain proof of card use for eligible expenses within 60 days, to prevent your card from being suspended.
How can I use the card for Over-the-counter drugs and medicines now covered through my Medical FSA?
You can use your Take Care® debit card in the pharmacy for over-the-counter drugs and medicines that have been approved by the IRS. Keep in mind that you will need to submit your cash register receipt, which includes the date of purchase, product description, and the amount of the expense for each OTC purchase. Another option available is through the internet website www.drugstore.com. From this site you will go to “Pharmacy”, then choose “Pharmacy Resource Center”. Under FSA eligible products, you can also purchase these items with your Take Care® debit card and you will not be required to submit these receipts. Additionally, anytime you use your Take Care® debit card at Walgreens®, WalMart, & Sam’s Club, your transactions will automatically adjudicate without the need of submitting receipts later. This is because these providers have loaded all UPC codes for all FSA eligible items as well as prescription drugs.

How can I sign up for the Take Care® Debit Card?
Contact your employer’s Personnel or Human Resources department to see if the Take Care® debit card is offered, or contact EBS/Atlanta. You can also order cards at anytime during the year through your employee account at www.myflexonline.com
What should I do with my Take Care® Debit Card , once my funds have been depleted?
You will be issued a new card at the beginning of the next plan year if you reenroll into the Flexible Spending Account. You can even order additional cards for a spouse or dependent by submitting the Dependent Card Request Form or going online to www.myflexonline.com. The first additional card is free, all cards after that are $5.00! Replacement cards after the first additional card are also available for $5.00.
How can I check my account balance on my account?
Your account balance and your Take Care® Debit Card transactions are detailed on EBS/Atlanta’s website at www.ebsatlanta.com, where you can connect to your employee account on www.myflexonline.com.
If funds are remaining in my Flexible Spending Account after the Plan Year ends, how do I receive my remaining balance?
If you elect to participate in the next plan year you will receive a new Take Care® Debit Card at the beginning of that plan year. The new Take Care® Debit Card will access any prior year funds during the applicable grace period outlined in your Summary Plan description.
